Practice Assessment
We review your patient panel to identify Medicare patients with two or more chronic conditions. You receive a clear picture of how many patients qualify and the estimated monthly revenue your practice can generate.
The Process
We manage every step of the CCM process — from identifying eligible patients to submitting CPT codes to your billing department — so your practice earns consistent monthly revenue without adding work to your team.
Get StartedWe review your patient panel to identify Medicare patients with two or more chronic conditions. You receive a clear picture of how many patients qualify and the estimated monthly revenue your practice can generate.
Our team contacts eligible patients, explains the CCM program, and obtains the required Medicare consent. Your staff does not need to manage outreach — we handle it entirely.
Each enrolled patient receives at least 20 minutes of non-face-to-face care coordination per month. This includes medication reviews, appointment follow-up, care plan updates, and chronic condition monitoring.
All care coordination activity is documented in compliance with Medicare requirements. We maintain accurate time logs and clinical notes so your practice is always audit-ready.
We apply the correct CPT codes (99490, 99439) and submit them directly to your billing department. Your billing team processes the claims under your NPI so revenue flows to your practice.
You receive regular reports on enrollment numbers, care activity, and revenue generated. As your enrolled patient count grows, so does your monthly CCM income.
